For us to be able to implement our Public Health and Mass Dispensing Plans, assistance from our many volunteers is required. Volunteers offer their help in a disaster should one occur, and they are trained to be most helpful in difficult times. If you are interested in becoming a volunteer for our emergency public health response team, please complete one of our forms either on-line or download paper form to sign up as a volunteer. We need non-medical as well as medical volunteers. If you have a medical license such as an RN or are a physician, please also complete our Medical Volunteer Data Form (2nd page of volunteer form).
If you would rather mail the form to us, click here to download:
*Current registered volunteers are encouraged to update their contact information regarding address, phone number, email, and credentials as they change.
It is also essential that individuals take the time prepare themselves for a pending disaster as best they can at their own homes. One credible website that contains good information on personal and family preparedness is www.ready.gov
You can pre-register for your vaccine through “Dispense Assist “ when an actual need arises to receive prophylactic antibiotics. Use the link to the right called "LINK TO DISPENSE ASSIST" to open the Dispense Assist utility in another window; follow instructions provided on the website. You will print a voucher which you will bring to the dispensing site.
